Azelaic Acid: Why Prescription Strength Is Different From What's in Your Serum

Azelaic acid has become one of the more talked-about skincare ingredients in recent years. It shows up in OTC serums, toners, and moisturizers, often marketed for acne, dark spots, and sensitive skin. The ingredient has genuine clinical evidence behind it. But there's a meaningful gap between azelaic acid at the concentrations available over the counter and azelaic acid as a prescription-strength treatment — and understanding that gap explains why so many people find OTC versions underwhelming.

What is azelaic acid?

Azelaic acid is a dicarboxylic acid that occurs naturally in grains. It has a genuinely unusual profile for a single ingredient — it addresses acne, hyperpigmentation, and rosacea through distinct mechanisms, making it one of the most versatile actives in clinical dermatology.

At the right concentration, it:

  • Inhibits tyrosinase — the enzyme central to melanin synthesis — reducing hyperpigmentation and melasma

  • Has direct antibacterial activity against C. acnes — the bacterium driving inflammatory acne

  • Reduces follicular keratinization — the process behind comedone formation

  • Has significant anti-inflammatory properties — reducing the redness and sensitivity associated with rosacea and inflammatory acne

  • Is well tolerated across skin tones, making it appropriate for patients where more potent actives carry PIH risk

The concentration problem

OTC azelaic acid products typically contain 5–10% concentration. Prescription-strength azelaic acid is 15–20%.

This isn't a minor difference. Clinical studies on azelaic acid for acne and melasma are conducted predominantly at 15–20% concentration. The efficacy data that makes azelaic acid a credible clinical treatment is based on prescription concentrations. OTC versions produce some benefit — but the clinical evidence base doesn't fully transfer to lower concentrations.

Acne

Azelaic acid vs other brightening actives

vs. Vitamin C (ascorbic acid) Both inhibit melanin production but through different pathways. Vitamin C is an antioxidant that also has tyrosinase-inhibiting properties. Azelaic acid is typically more potent for tyrosinase inhibition at equivalent concentrations, and better tolerated in sensitive skin. They can be used together.

vs. Niacinamide Niacinamide reduces melanin transfer between melanocytes and keratinocytes (a different mechanism from tyrosinase inhibition). Complementary to azelaic acid rather than competing — many prescription protocols use both.

vs. Hydroquinone Hydroquinone is more potent for severe hyperpigmentation and melasma but requires clinical oversight and cycled use. Azelaic acid is the preferred first-line option for patients with medium to darker skin tones, and a useful concurrent active in protocols that include hydroquinone.

vs. Tranexamic acid Tranexamic acid works upstream of azelaic acid in the melanin production pathway — blocking the UV-induced signal that triggers melanocyte overactivation. The two are highly complementary and are frequently combined in prescription melasma protocols.

Tolerability across skin tones

One of azelaic acid's most clinically valuable properties is its tolerability across the full Fitzpatrick spectrum. Unlike some highly effective actives that carry meaningful PIH risk in darker skin tones (certain retinoid concentrations, strong chemical exfoliants), prescription azelaic acid is generally well tolerated even in Fitzpatrick V and VI.
